The eligibility for the services would depend on the qualifications for our Congregate Living Health Facility services. At Gimag Transitional Care, in order to qualify for our Congregate Living Health Facility, your loved one must have:
- Be a resident of Tracy, California.
- Be at least 18 years of age.
- Have a medical condition or illness that requires skilled nursing care on a recurring, intermittent, extended, or continuous basis.
- Be unable to live independently due to your medical needs.
- Be able to benefit from the services provided by a CLHF.
To apply for CLHF services, you will need to complete an application and provide documentation of your medical condition and need for care. You will also need to have a physician’s order for CLHF services. Once you have applied, your eligibility will be assessed by a CLHF team. This team will include a registered nurse, a social worker, and other healthcare professionals. The team will evaluate your medical needs and your ability to benefit from the services provided by a CLHF. This assessment will help the CLHF staff to develop a personalized care plan for you.
